Ayushman Bharat (PM-JAY)

Ayushman Bharat PM-JAY is the worlds largest health insurance scheme fully financed by the government. It provides a cover of ?5 lakh per family per year for secondary and tertiary care hospitalization across public and private empanelled hospitals in India.

• Cashless and paperless access to services
• Covers up to 3 days of pre-hospitalization and 15 days of post-hospitalization
• All pre-existing conditions covered from day one
• Separate ?5 lakh cover for senior citizens aged 70+ (Vay Vandana Card)

• Rural families living in kucha houses or landless households dependent on manual labor
• Urban households belonging to identified occupational categories (beggars, ragpickers, domestic workers, etc.)
• All senior citizens aged 70 years and above

• Aadhaar Card
• Ration Card / Family ID
• Proof of Residence
• Income Certificate

1. Visit pmjay.gov.in
2. Click on "Am I Eligible"
3. Enter mobile number and OTP
4. Search by Aadhaar/Ration Card
5. If name is in list, visit nearest CSC or empanelled hospital for KYC
